The general rules first, then every house. Everything not confirmed on a page we could read says “check”.

General rules

Age

18 in most states. Baden-Württemberg: 21 for entry. Bavaria: playing from 21; entry from 18 with photo ID and registration (Bad Reichenhall page); secondary sources add a companion aged 21 or older.

ID

Every house checks and registers an ID at entry. Personalausweis and passport are accepted at most houses (Bad Ems names the Personalausweis only). Driving licences are accepted in Bad Reichenhall (EU licence) but not in Bad Homburg.

Dress

Slot halls are usually neat casual. The classic hall can ask for more: Baden-Baden requires a jacket for men; Bad Kissingen reportedly too.
